Psoriasis

Psoriasis is a chronic inflammatory skin disease for which there is no definite cause or treatment. In addition, this disease is related to the immune system. Its early or late complications affect the quality of patients’ life, so timely diagnosis and correct treatment of this disease will reduce the complications and control the disease progression.

Symptoms: 

Psoriasis is a chronic inflammatory disease with symptoms such as thickening of the skin, scaling, and redness. This disease is related to the immune system and the rate of reproduction and growth of skin cells increases disproportionately, as a result, several layers of dead cells accumulate on the surface of the skin, which form the same crusts as psoriasis. These layers mostly appear around the scalp, elbows, and knees and are accompanied by itching and sometimes pain. 

There are different types of this disease, of which 5 types are more common: plaque psoriasis (the most common type), droplet, inverse, vascular, and erythrodermic.

Treatment method in the hospital:

• Currently, there is no definitive treatment for this disease, but there are many medications that can be used to control psoriasis. Due to the different nature of psoriasis in different people, the treatment method for each person will be different depending on the age, sex, job, extent of the disease, and many other factors.

• Topical corticosteroids are usually the most common treatment used for psoriasis, reducing inflammation and skin irritation. The important point about the use of topical corticosteroids is that often after some time, the initial desired response is no longer achieved, while there is also the risk of thinning of the skin due to the use of these drugs.

• Irradiating ultraviolet light to the skin regularly and under medical supervision. Treatment is done at the skin clinic or at home with a phototherapy device. After taking a drug called psoralen, the patient is exposed to ultraviolet rays (UVA) which is called PUVA treatment. Today, in addition to PUVA, more specific ultraviolet spectrums such as UVB are also used in the treatment of psoriasis.

Systemic drugs:

 • Methotrexate: It is the first drug that is used for this purpose. This drug reduces the proliferation of skin cells and is prescribed together with folic acid.

• Vitamin A derivatives: are known as retinoids, such as neotigazone, which should be taken with plenty of water, and the type of oil used should not be solid. Due to the possible effect of this drug on the increase of blood lipids and liver enzymes, it is mandatory to check liver enzymes and blood lipids during the treatment period. During the treatment period and after stopping it, avoid pregnancy for up to 3 years.

Biological drugs:

Are usually prescribed for moderate to severe psoriasis and psoriatic arthritis that does not respond to other treatments?

• How to reduce itching in psoriasis?

• Minimize the scales by using emollients.

• Cold showers and cold compresses can also relieve itching.

The length of stay in Iran: proportional to the length of the treatment period, at least 10 days
