Fetoscopic Laser Surgery

Fetoscopic laser surgery is a selective treatment for twin-to-twin transfusion syndrome (TTTS), performed in the operating room using a laser and fetoscope. This syndrome occurs in mothers with twin pregnancies, where the twins share a common placenta. In TTTS, this connection is unbalanced, meaning one fetus receives blood from the other, leading to symptoms over time and causing both fetuses to become ill. It is important to note that in cases where the vessels of the placentas of the two fetuses are fused and do not respond to treatments such as amnioreduction, fetoscopic laser surgery is performed between 18 and 28 weeks of gestation to prevent complications from blood insufficiency to the fetuses and fetal demise.
At Tehran University of Medical Sciences, more than 203 cases of fetoscopic laser surgery have been conducted to date, with this technique proving effective in over 90% of cases for maintaining one twin and over 60% for preserving both twins.
